Lancashire County Council is seeking an experienced Social Worker to join their Children with Disabilities Team. This is a fantastic opportunity for a dedicated professional to support children and young people with complex needs, ensuring they and their families receive the best possible care and support.
Key Responsibilities:
- Assess the needs of children with disabilities and their families.
- Develop and implement care plans and support packages tailored to individual needs.
- Collaborate with other professionals and agencies to provide holistic support for children and families.
- Provide ongoing care management and review care plans regularly to ensure they meet changing needs.
- Undertake statutory visits, risk assessments, and mental capacity assessments where necessary.
- Prepare and present court reports and other relevant documentation.
Requirements:
- Qualified Social Worker with experience in children's services, preferably working with children with disabilities.
- Demonstrable experience in assessing complex needs and implementing care plans.
- Knowledge of legislation relating to children with disabilities, including the Children Act and Care Act.
- Ability to manage caseloads of between 15 and 22 (depending on the function).
- Excellent assessment, communication, and report-writing skills.
- Ability to work in a hybrid environment and collaborate with the team when required.
